Health Planner Page (Introduction)
The information you record in your activity diary will help to design an action plan to meet your personal needs. This information will help you to get the most out of your diary.

Week one:
In this space indicate the date when you start the activity. This must be for 1 whole week. Do not put in activities you have done, for example, over 2 or more weeks.

Activity:
In this column, write down the type of activity you have done. This does not have to be exercise, it can be any type of physical activity that you have done, for example, washing the car, walking the dog, or mowing the lawn.

Time:
Write down how much time you have spent doing this activity. If for example you have taken 18 minutes to mow the lawn, then write 18 in the box, if it has taken you 20 minutes then circle 20.

Total week's/minutes:
Calculate the total minutes form everyday and put the grand total for the week in this box.
